Getting from Panama to Colombia overland is not an easy feat due to the Darian Gap blocking the way. Not only is there no road through the Darian Gap but it is also a very dangerous place. The main option for getting to Colombia from Panama is to catch a boat. The standard trip is 5 days from somewhere on the Caribbean side (such as Porvenir or Portobello) to Cartagena. It is normally done in 5 days with some stops in the idilic San Blas islands. hostels in Panama city will often have information on captains willing to take people across. You may have to get a group of people together to charter the boat though.
